File a Complaint

If you have had your civil rights violated, parental rights terminated, or children's rights abused, there IS something you can do. Foundation for Court Reform (FCR) will review your case and provide needed assistance.

When contacting us, these forms must be completed, signed and notarized before they are sent to FCR. The first form is the FCR Complaint Form. Please print legibly, fill it out completely, and use additional pages as necessary. The second form is a RELEASE that allows us to forward your complaint to a research and review committee.

Because of minor children, we need you to sign a RELEASE for yourself and each minor child "directly" involved in the complaint. You may need to make extra copies of this form for each minor child involved.

In addition, include a copy of your "certified copy" of "any and all" Appearance Dockets or Minutes made in your court file(s) from each district, county, appellate, supreme, and federal court(s) from all States that apply in your case. Be sure to keep your original copies.

As you complete the form, write out in chronological order the sequence of events as they occurred and a brief summary explaining the problems that transpired and the people involved in covering up crimes committed in your case. Include their contact information, place of employment, position, address, and phone number. It may be helpful to get a certified copy of your complete court file to include the Minutes or Docket Appearance and the court hearing transcript(s) for your review.
